Community Health Hub

In Community Health Hub, you are tasked with transforming a community's behavioral health system. As a grant recipient, you must plan, develop and implement a new Community Behavioral Health Center (CCBHC) based on the CCBHC Certification Criteria. You will have to provide comprehensive outreach, screening, assessment, treatment, care coordination, and recovery supports based on a needs assessment, and support recovery from mental illness and/or substance use disorders (SUD) regardless of an individual's ability to pay. The grant proposed budget cannot exceed $1,000,000 in total costs in any year of the proposed project. Annual continuation awards will depend on the availability of funds, grantee progress in meeting project goals and objectives, timely submission of required data and reports, and compliance with all terms and conditions of award.
